In addition to employer groups, the Future Guard plan may be available to other types of group sponsors recognized by state insurance law, including labor unions, trade and professional association groups, and credit unions.
All enrollments are handled by direct mail and the plan is identical in all states.
Members and their spouses age 65 or less may apply by completing a simplified application with only a few medical questions. Members and spouses age 66 or more are subject to full health underwriting. Family members may also apply, including siblings, parents, parents-in-law, grandparents, grandparents-in-law, subject to full health underwriting.
Insurance Department filings allowing solicitation of residents are required in certain states. Filings will only be done where there is a substantial population of members.
