APPLYING IS EASY:

1. Download and review the Future Guard Brochure & Outline of Coverage. Premium charts may be found on pages 10 and 11.

2. Download and complete your Enrollment Forms. Be sure to complete each section of the application carefully, answering each question, and sign and date your application. We cannot process your application unless all information is complete. Payroll deduction is not available on this program. You can be billed on a quarterly, semi-annual, or annual basis or you can set up a monthly automatic bank draft payment.

3. Mail the form(s) to the Plan Administrator:

Or fax the form(s) to (858) 451-7051.

No payment is due until after your coverage is issued. If you are accepted, we will send your Certificate of Insurance to your home address by mail.

4. If you would like to order hard copy enrollment kits to be mailed to you or to a family member, please call (888) 287-8810 24/7, or e-mail us.

If you have any questions about the plan itself or need help with your application, please call the Plan Administrator toll-free at (800) 603-7999 Monday through Friday between 8:30 a.m. and 5:00 p.m. Pacific Time, or e-mail us. An expert long-term care specialist will be happy to help you.
